Each participant must complete their own registration form.
Each email address can be used to register only one participant.
It is not possible to simply change the name in an existing registration – this will invalidate the original registration. Thank you for your understanding.
Hotel ALDIS is fully booked – accommodation there is available ONLY for VIP guests.
Important Note
To receive CME credits for attending the educational event, you must enter your ČLK personal ID number in the registration form.
Please ensure that the number is entered correctly.
Credits will be assigned to your personal ČLK account automatically, based on the list of participants submitted by the organizer, approximately 14 days after the event.
